Statement regarding license and image of persons *

I certify that I have obtained the appropriate consent and licenses to publish the photos attached to the form. I grant the Zbigniew Herbert Foundation an unlimited time and territorial license to publish and reuse, and to sublicense all works that I have attached to this form. The license applies to publication on the Zbigniew Herbert Foundation website, in promotional materials and digital processing of the works. I agree to the publication of images of people who are in the photos.

Thank you for taking part in the Gift a Poem campaign. Before filling out the form below, please read through the following general rules that will help us process your reports efficiently:

The reports that ASPnet National Coordinators send in will allow us to establish a record of the campaign in each country, so please write them in such a way that we will be able to publish them on the campaign’s website. Please submit your reports at the earliest opportunity, no later than on April 23rd.
For ease of publication, each report’s length should be up to 2500 characters including spaces.
Please attach photos documenting the campaign in your country – ideally 6 to 12 files of up to 3MB in size.
If you have video documentation of the campaign, please upload the files to YouTube or Vimeo and attach the links in your report. We will then embed the videos on the campaign’s website.
